Care Coordinator

Title: Care Co-ordinator (Homecare)
Location: Cheltenham
Contracted Hours: 40 hours per week, Monday to Friday, 8:30am – 5:00pm (30-minute unpaid break)
Salary: £25396.80 per annum bonus

The Role:
As a Care Coordinator you will be required to ensure the smooth running of the CQC Regulated branch supporting individuals in their own homes maintaining its excellent reputation, seek new opportunities, ensuring that the care and coordinating team follow company policies as well as national and local guidance.
 
Main Duties and Responsibilities:

  • To ensure the branch continues to grow and develop its homecare service.
  • Work alongside the Registered Manager to drive the team and branch towards achieving weekly KPIs. Key areas of focus will include optimizing new staff recruitment, increasing the number of client packages, and developing our private funding customer base.
  • Ensure that the coordinating team create travel effective rotas in advance and that customers have a consistent high-quality service from people they know.
  • Effectively manage team rosters, matching skills to specific client requirements.
  • Report and record any safeguarding concerns to the Branch Manager.
  • Attend training and expand your personal development, you will need to have the ability to use reflective practice and that learning logs are completed so that lessons can be learnt and improve overall service delivery for the branch
  • Approve and keep a record of holidays and ensure there is appropriate staffing levels to ensure capacity.
  • Monitor sickness and absence of staff and work with staff to improve where their attendance is below the expected standard.
  • You will need to ensure that the homecare service works in an outcome focus way that is person centred to the individual customer. Ensuing that support plans a written to a high standard and that risk assessments are carried out and reviewed in line with the company’s policy. Mentoring to staff responsible for care plans and risk assessments on how this should be carried out.
 
The Ideal Candidate Will Have:
  • Relevant experience in similar role (ideally with NVQ Level 3 in Social Care)
  • A full UK Driving License

We are looking for a dedicated Healthcare Assistant to provide complex care to a client in Wiltshire. This role offers bank shifts on an as-needed basis, primarily 12-hour waking nights, with some day shifts also available. A full UK driving licence is essential.
 
The main duties and responsibilities include:
  • Remaining awake and alert for the entirety of the shift.
  • Completing regular checks for health and welfare.
  • Supporting the customer with their personal care needs.
  • Supporting the customer to meet their outcomes.
  • Administering medication.
  • Use of moving and assisting equipment such as ceiling track hoists, electric wheelchairs, through the floor lifts, toto’s etc.
  • Encouraging independence where possible.
Knowledge of spinal injury, autonomic dysreflexia, diabetes, stoma care and catheter care is desirable but if not, we can provide full training.

You will enable customers to maintain their independence and quality of life in their own homes. Key Responsibilities:
  • When required liaise with Social Workers, District Nurses and other agency professionals within the multidisciplinary team.
  • Contribute towards communicating changes of requirements.
  • Ensure that you work within the company policies and procedures.
  • Attending training and expanding your personal development
  • Assist customers as appropriate with their personal needs, such as dressing, washing, meal preparation and access to their community as detailed in the Care Plan.
  • When working within a customer’s home setting you may be assisting customers with, or where necessary carry out light housework, such as dusting, sweeping/ vacuuming carpets, living rooms/ kitchens/ bathrooms and toilets, polishing furniture using dusters/ mops/ equipment, washing and ironing clothing and bed linen, preparing and cooking a meal.
  • Following the outcome focused model, encourage customers to undertake personal care and tasks which they are capable of, recognising that their abilities may change, in some circumstances, on a day-to-day basis

The Role


We are currently seeking a Clinical Psychologist in Gloucestershire  to provide psychological assessment, formulation and interventions to individuals with diverse needs including learning difficulties, autism spectrum conditions, eating disorders and behaviours that challenge. This would be in a range of clinical settings, such as supported living, across our services in Wiltshire and potentially neighbouring counties.

Key Responsibilities


Support multidisciplinary teams with psychological perspectives and interventions.
Formulate and implement treatment and management plans based on clinical formulation and evidence-based practice.
Offer consultation and advice to staff teams, families, and carers to support the psychological care of service users.
Contribute to risk assessment and care planning.
Maintain up-to-date and accurate clinical records in line with legal, ethical, and professional standards.
Provide high-quality psychological assessments, formulations, and evidence-based interventions to individuals and groups
 
Requirements

Ability to work flexibly across different services as required
A valid UK driving licence and access to your own vehicle
Demonstrable experience working with complex mental health needs, ideally in a social care setting
A valid registration with HCPC
Excellent interpersonal and communication skills
Doctorate in Clinical Psychology or equivalent
