Homecare Coordinator
Location: Chippenham, Wiltshire
Hours: 8:30-17:00 Monday to Friday

Retain healthcare are currently recruiting for a homecare coordinator. This position will be to provide support for the homecare contract across the whole of Wiltshire, supporting adults who have been discharged from hospital and re-establishing their independence in their own homes.

This role will be office based, although you will be required to go out into the community for visits or assessments. You will also need to go out into the community when required, to cover for our support workers, if they are unable to attend a visit. This role would be suited to someone with experience of working in care, preferably at a team leader level.

Our criteria for a home care coordinator includes:

· A full UK driving license with access to a vehicle.

Our Reablement service will allow our clients the option of returning to the comfort of their own home to recover from an accident, illness or operation. Our Reablement Support Workers will work with people to regain their confidence and skills required to live as independently as possible in their own home, working towards goals set out upon discharge from hospital.

These goals can range from support in preparing meals and household tasks to personal care and will be tailored around improving quality of life for those we work with. Progress is reviewed regularly and care plans are tailored accordingly, in order to monitor and adapt to individual support needs. A final review is then carried out at the end of the reablement period to ensure the support package is either no longer required or will move onto a longer term solution.

    Main Duties and Responsibilities:
    • To work alongside and assist the Branch Manager and Office Team
    • To have a full understanding of the CQC requirements for Domiciliary Care Services and ensure that the organisation exceeds these expectations
    • To take responsibility for the organisation and maintenance of Care Plan files in their area. To highlight to team members when our standards have not been reached.
    • Work in a flexible manner that meets the needs of the service
    • Carry out new service assessments ensuring that an agreed Care Plan and Risk Assessments are in place before the service commences, unless, in emergency cover agreed by the department Manager
    • To ensure that every service user has a Support Plan and Risk Assessments that are regularly reviewed and detailed to meet their individual needs
    • You will ensure that the homecare service works in an outcome focus way that is person centred to the individual customer. Ensuing that support plans a written to a high standard and that risk assessments are carried out and reviewed in line with the company’s policy. Mentoring to staff responsible for care plans and risk assessments on how this should be carried out.
    • Maintain and always ensure that office records and notes are accurate and up to date, including the correct storing of information and adherence to the Data Protection Act
    • Report and record any safeguarding concerns to the Branch Manager.
    • Communicate regularly and when required with customer and their family; informing next of kin when an incident has occurred if appropriate
    • Contribute towards communicating changes to rotas and client requirements within the community
    • Ensure that carers adhere to company policies and procedures
    • Identify any further training needs of carers information observed or service users’ feedback
    • Ensure that the department operates at an excellent rating with the Care Quality Commission and that all organisational objectives are fully met by their team
    • Perform “On-Call” duties: (on a rota basis as necessary) dealing with carers’ sickness and cancellation of calls by arranging cover; custodian of keys and codes; associated paperwork; keeping head office informed regarding communicating outcomes. To ensure that the On-Call rota is covered as a priority.
    • Ensure the branch Keep effective relationships with the staff, have an understanding of their work requirements. Ensure the team roster effectively, skill matching to specific needs are managed effectively.

    We at Retain Healthcare are looking for confident individuals to work in a complex care package in Chippenham.
    This package has two shift patterns, 08:00-20:00 and 20:00-08:00. Please note that night shifts are waking nights.
     
    The main duties and responsibilities include:

    • Remaining awake and alert for the entirety of the shift.
    • Completing regular checks for health and welfare.
    • Supporting the customer with their personal care needs.
    • Supporting the customer to meet their outcomes.
    • Supporting the customer with daily activities such as shopping, hobbies, attending appointments, accessing the community and keeping the home up together with domestic tasks.
    • Administering medication.
    • Use of moving and assisting equipment such as ceiling track hoists, electric wheelchairs, through the floor lifts, toto’s etc.
    • Encouraging independence where possible.
    Knowledge of spinal injury, autonomic dysreflexia, diabetes, stoma care and catheter care is desirable but if not, we can provide full training.

    Requirements:
    • Previous care experience necessary
    • 25 years old and over
    • Must hold a UK driving license

    Retain Healthcare are developing a brand new service, and we are looking for the very best support workers in Wiltshire to work within a specialist supported living setting in Chippenham. You will be working alongside a team of experienced and dedicated staff who specialise in supporting young adults with an Autistic Spectrum Conditions and Eating Disorders.  You will be supporting people with a therapeutic approach that enables them to live a fulfilling life.

    This team will receive the very best training and development to ensure that they are skilled in supporting the people we support.     Location:
    The service is situated in Chippenham.
    Your duties will be assisting with all aspects of daily living including personal care, medication and supporting to attend healthcare appointments.
