Retain Healthcare Reablement Service

Reable, Rebuild, Retain Independence

At Retain Healthcare, we believe the right support at the right time can help people regain confidence, rebuild daily routines, and remain independent at home.

Our reablement service is designed to support people following illness, hospital discharge, a fall, a change in mobility, or a period of reduced confidence. Rather than simply “doing for” someone, our trained care teams work alongside each person to help them do more for themselves, safely and at their own pace.

Helping People Regain Confidence at Home

Reablement is short-term, focused support that helps people rebuild the skills and confidence they need for everyday life. This may include getting washed and dressed, preparing meals, moving safely around the home, managing medication routines, or reconnecting with the local community.

Our approach is person-centred, practical, and outcome-focused. We work with individuals, families, health professionals, and commissioners to create a clear support plan built around what matters most to the person.

Our Approach

Reable
We support people to regain ability, confidence, and independence after a setback.

Rebuild
We help rebuild routines, daily living skills, mobility, and confidence in the home and community.

Retain Independence
Our aim is to reduce long-term dependency where possible, supporting people to remain safe, well, and independent in their own homes.

What We Can Support With

Our reablement teams can assist with:

	Personal care and daily routines
	Safe mobility around the home
	Meal preparation and nutrition
	Medication prompts and support
	Confidence-building after hospital discharge
Falls prevention and home safety awareness
	Support to access the community
Light domestic tasks linked to independence
	Working towards agreed personal goals

Why Choose Retain Healthcare?

Retain Healthcare provides regulated care and support across the South West, with experienced local teams who understand the importance of safe, responsive, and compassionate care.

We combine high-quality care delivery with a strong training culture through Retain Skills, ensuring our teams understand the principles of reablement, dignity, safeguarding, moving and assisting, and person-centred support.

Our service is built around:

	Outcome-focused care planning
	Skilled, values-led care staff
	Local management and responsive support
Strong communication with families and professionals
A focus on independence, not dependency
Flexible support from short visits to more complex packages

Supporting Hospital Discharge and Preventing Readmission

Timely reablement can play a vital role in helping people return home safely from hospital and reducing the risk of readmission. Our teams can provide practical support during the crucial first days and weeks at home, helping people rebuild strength, confidence, and routine.

Where appropriate, we can also work alongside assistive technology and monitoring solutions to support safer independence and provide reassurance to families and professionals.

A Service Built Around You

Every person’s journey is different. That is why our reablement support starts with understanding the individual: what they could do before, what has changed, what matters to them, and what they want to achieve.

Whether the goal is making a cup of tea independently, walking safely to the bathroom, preparing a meal, or getting back out into the community, we focus on small, meaningful steps that make a real difference.

Reable, Rebuild, Retain Independence

At Retain Healthcare, our mission is simple: to help people regain control, rebuild confidence, and retain the independence that matters most.
